Diagnosis of folliculitis keloidalis is primarily clinical, involving the identification of follicular papules, pustules, and scars on the occipital scalp. Dermatologists diagnose AKN by examining the back of the scalp and neck for acne-like breakouts, scars, hair loss, and signs of infection. Questions about hair-cutting practices and the use of helmets or other headwear are common during the diagnosis.

Laboratory Tests

If an infection is suspected, a swab of a pus-filled bump may be taken and sent to a lab for testing. A biopsy may be performed if the diagnosis is unclear, and a culture might be ordered to identify the type of infection and appropriate treatment.
